CMS published new guidance on requiring modifier JZ on claims that represent that there was no waste when the drug was administered.

This change applies to a single vial or single package drug or biologic.

Following this UHC has updated their own policy for various states where they have Community Plan.

Medical Billing and Coding: the following information is required on the claim

• CPT® or HCPCS code of the given drug
• JZ modifier
• The number of units administered
