The update applies to Medicare Advantage Plans, Community Plans, Commercial and Individual plans on Exchange.

What is changing: patients that need radiation procedures after DOS 1/1/26- providers please submit prior authorization requests with the correct coding.

The patients that had an approval before the start date of 1/1/26- no action is needed- UHC will update the system to reflect the information with the correct coding.

Providers: please make sure you are requesting the current correct codes.

To submit the prior auth request:

1. Go on UHC portal https://secure.uhcprovider.com/ on the left side hand tabs please choose Prior Authorizations and Notifications
