Please note that Turquoise Care recently updated its taxonomy crosswalk. Please log into New Mexico Medicaid portal to identify the proper Taxonomy code.

What is required?

• Zipcode
• Proper Taxonomy code

What will happen if the info is missing or incorrect?

1.UHC will reject claims if the taxonomy code is missing.

Which type of provider does this apply to?

2.UHC will deny if the Taxonomy code does not match the zipcode of the provider.

All provider types- both institutional and professional.

Please note that the zipcode needs to match the physical location where the services where performed.
