How to request an auth?

• Sign into UHC portal
• Click on Prior Authorizations and Notofications, from the left-hand tab, then select “create a new request”.
• Select Oncology- enter the members info and plan and then you will be redirected to Optum website.

What happens is you have an existing auth and need to modify the auth?

• Even if the request was granted before 6/1/26- provider needs to submit the auth request again
