• Commercial- all states
• Medicare Advantage- all states
• UHC commercial plans in the following states: New York, New Jersey, PA, TX, Arizona (does not include the full list) -please check with your UHC Community Plan that you accept.in your office/facility.
• UHC- exchange plans in the following states: New York, New Jersey, Alabama, FL, Colorado (does not include a full list) please check with the UHC individual exchange plan for your state.
