What you should know?

• The Michigan Collaborative for Type 2 Diabetes (MCT2D)- was a pilot program that will end on 1/1/27
• Under this program the prior authorization process was bypassed and the patient qualified with Type 2 Diabetes DX
• Once the pilot program will end the patients will have to meet the UHC standard CGM coverage criteria – prior authorization will be required
• This change applies only to Commercial plans

Coverage of the current CGM

• If your office participates in this program and the patient receives the CGM under the current program before Dec 31st 2026, then you will not need to complete prior auth for 12 months or 12/31/27, whichever comes first.
