NY/CA UHC CommunityHealthPlan: updated info for Xolair and Synagis auth, 04/01/2021

Starting 04/01/2021- the providers will need to request an authorization for Xolair and Synagris if they plan on billing through patient medical benefit. (Community Health Plan)

This change applies to providers in CA and NY (community health plans).

These requirements will apply whether members are new to therapy or have already been receiving these medications.

UHC will honor existing authorizations for Xolair until the end date on the authorization or the date the member’s eligibility changes.

Providers don’t need to submit a new prior authorization request for members who already have an authorization for Xolair on April 1, 2021.

You can submit prior authorization requests in the following ways:

  • Online: Use the Prior Authorization and Notification tool on Link. Go to UHCprovider.com/paan.
  • Call: Use the Provider Services phone number on the member’s health care identification card.

NY UHC (Medicaid) Orthotics: added codes to prior auth list, 04/01/2021

Starting 04/01/2021 the following codes will be added to NY Medicaid list of codes that require an authorization.

This change applies to the following UnitedHealthcare Community Plan members:

  • UnitedHealthcare Community Plan for Families (Medicaid)
  • UnitedHealthcare Community Plan Wellness 4 Me (HARP)
  • UnitedHealthcare Community Plan Essential Plan (EP)
  • UnitedHealthcare Community Plan ChildHealthPlus (CHP)

Administrative Guide Medication Sourcing Requirement for Xiaflex – Apr. 1, 2021
As a reminder, the 2021 Care Provider Administrative Guide includes information on medication sourcing requirements for in-network providers. The sourcing requirement for some specialty medications apply to all outpatient care providers (referred to as Medication Sourcing), while other sourcing requirements only apply to outpatient hospital providers (referred to as Medication Sourcing Expansion). Xiaflex will be placed under Medication Sourcing Program that applies to all outpatient care providers, effective Apr. 1, 2021. UHC have contracted specialty pharmacies for the distribution of these specialty medications. UHC participating specialty pharmacy providers give fulfillment
and distribution services to meet the needs of UHC members and UHC care providers. Effective Apr. 1, 2021, you must acquire Xiaflex from US Bioservices, unless otherwise authorized by us. The requirement does not apply when Medicare or another health benefit plan is the primary payer and UHC is the secondary payer. Contact your local UnitedHealthcare network management representative if you have any questions.

Riabni™(rituximabarrx)Jun. 1, 2021 – Rituximab is for the treatment of several
cancer and non-cancer related conditions, including: immune thrombocytopenic purpura,
pemphigus vulgaris, Wegener’s granulomatosisrheumatoid arthritis.

• Adding notification/prior

• Preferred products:
Ruxience® and Truxima®;
Rituxan®, Rituxan Hycela®
and Riabni™ will be nonpreferred.
