The process- to make it easier- providers should contact Evernorth directly instead of Cigna.

How to request an appeal?

Providers can fill out a form – Behavioral Appeals Cover Sheet and attach it with any additional information.

If providers have questions regarding this form, please call Evernorth at 800.926.2273.

What type of appeals should be sent over to Evernorth?

• Timely filling claim denial
• Medical Necessity
• Inpatient level of care
• Administrative denials for mental health services, substance use disorder services, and any other services covered under behavioral health benefits.
