• Prior authorization will be removed for the following codes: G0281 and G0282
• Effective for any dos 9/1/26 and thereafter

The affected UHC plans are:

• UHC Community Health Plans in the following states: NY, AZ, CO, FL, IN, NJ, NW, TX

If your state is not listed above this does not apply to the providers in other states.
